Annual Information Report


The Annual Information Report will now be submitted electronically via this website. Please familiarize yourself with the instructions below and then click here to go to the on-line form.

  1. first register (creates a user name and password for your school) by clicking the “please click here to register” link on the login page and follow the directions 
  2. complete and submit Annual Information Report and Substantive Change 
  3. ONCE THIS PORTION IS SUBMITTED IT WILL RETURN TO THE MAIN MENU to proceed to the next link
  4. review/update and submit the evaluator list (even if no changes are necessary for evaluator information
  5. click the Logout link 
You can log back in to the website at any time to view your submitted information. If you have any questions, please email cdonoghue@neasc.org
